Actinic Keratosis

What is it?

Actinic keratosis represents a premalignant cutaneous lesion characterized by dysplastic (abnormally developing) keratinocytes resulting from chronic ultraviolet radiation exposure. These lesions manifest as rough, scaly patches that develop on chronically sun-exposed skin and carry the potential for malignant transformation to cutaneous squamous cell carcinoma. The condition is also referred to as solar keratosis or senile keratosis, reflecting its association with cumulative sun damage over time.

Who is affected?

Actinic keratosis demonstrates a global prevalence of approximately 14% in the general population, with significantly higher rates in specific demographic groups:

  • Age demographics: Predominantly affects individuals over 40 years, with prevalence increasing substantially with age
  • Gender distribution: Higher prevalence in males due to greater occupational and recreational sun exposure
  • Ethnic predisposition: Markedly more common in fair-skinned individuals, particularly those with Fitzpatrick skin phototypes I and II (those who burn easily and tan poorly)
  • Geographic factors: Higher incidence rates in populations living closer to the equator and at higher altitudes
  • Occupational risk factors: Elevated prevalence among outdoor workers, farmers, construction workers, and athletes

What causes it?

The etiology of actinic keratosis involves cumulative DNA damage from chronic ultraviolet radiation exposure:

  • Primary pathogenic mechanism: UV-B radiation (280-315 nm) causes direct DNA damage and formation of cyclobutane pyrimidine dimers in keratinocytes
  • Contributing factors: Immunosuppression (either pharmacologic or age-related), fair skin phenotype, and genetic predisposition
  • Environmental influences: Geographic location, altitude, occupational exposure, and recreational sun exposure patterns
  • Cofactors: Previous history of skin cancer, chronic wounds, and certain chemical exposures

What are the clinical features?

Actinic keratosis presents with distinct signs that patients can identify:

Signs (objective, observable findings):

  • Rough, sandpaper-like texture (hyperkeratotic surface that feels gritty to touch)
  • Erythematous scaling patches (red, scaly areas ranging from a few millimeters to several centimeters)
  • Irregular borders (lesions with poorly defined, asymmetric edges)
  • Color variations (ranging from flesh-colored to pink, red, or brown)
  • Location pattern (predominantly on sun-exposed areas including face, scalp, ears, lips, dorsal hands, and forearms)

Symptoms (subjective, patient-reported experiences):

  • Pruritus (itching sensation, particularly when lesions become inflamed)
  • Tenderness or pain (especially when lesions are traumatized)
  • Bleeding propensity (easy bleeding with minor trauma or scratching)
  • Burning sensation (particularly with sun exposure or irritation)

How is it diagnosed?

Diagnosis of actinic keratosis relies primarily on clinical assessment:

  • Clinical evaluation: Visual and tactile examination focusing on characteristic morphology and distribution pattern
  • Dermoscopic assessment: Non-invasive examination revealing specific features such as follicular plugging and vascular patterns
  • Histopathological confirmation: Skin biopsy reserved for atypical presentations or lesions suspicious for malignant transformation
  • Differential considerations: Distinction from seborrheic keratoses (benign growths), squamous cell carcinoma (malignant transformation), and other hyperkeratotic conditions

What treatment options are available?

Therapeutic management employs both lesion-directed and field-directed approaches:

  • Lesion-directed therapies:

    • Cryotherapy with liquid nitrogen (most commonly used first-line treatment)
    • Curettage and electrodesiccation for isolated lesions
    • Surgical excision for large or atypical lesions
  • Field-directed therapies (treating entire areas of sun damage):

    • Topical 5-fluorouracil (chemotherapy agent for multiple lesions)
    • Imiquimod cream (immune response modifier)
    • Photodynamic therapy (light-activated treatment)
    • Chemical peels with trichloroacetic acid
  • Emerging treatments:

    • Ingenol mebutate gel (plant-derived topical agent)
    • Ablative laser therapies
    • Combination treatment protocols
