Herpes Zoster (Shingles)
What is it?
Herpes zoster, commonly known as shingles, represents a painful vesicular eruption caused by reactivation of the varicella-zoster virus (the same virus that causes chickenpox). Following initial chickenpox infection, the virus remains dormant in nerve ganglia (nerve cell clusters) for decades before reactivating along specific nerve pathways, producing a characteristic painful rash distributed in a dermatomal pattern (following the path of a single nerve). This condition manifests as clusters of fluid-filled vesicles (blisters) accompanied by significant neurological pain that may persist beyond rash resolution.
Who is affected?
Herpes zoster demonstrates distinct epidemiological patterns influenced by age and immune status:
- Overall incidence: Affects approximately 1 in 3 individuals (32%) during their lifetime
- Annual occurrence: 3 to 5 cases per 1,000 person-years in the general population
- Age-related increase: Incidence rises to 11.8 per 1,000 person-years in adults over 65 years
- Pediatric occurrence: Uncommon, affecting 0.74 per 1,000 person-years in children
- Risk factors: Advanced age, immunosuppression (cancer treatment, HIV, organ transplantation), psychological stress, and certain chronic conditions (diabetes, chronic lung disease)
- Gender distribution: Slightly higher incidence in females (13% higher risk than males)
- Recurrence rate: Second episodes occur in approximately 6.2% of patients
What causes it?
The pathogenesis involves viral reactivation triggered by declining immunity:
- Primary infection: Initial varicella-zoster virus infection causes chickenpox, typically in childhood
- Viral latency: Following chickenpox resolution, the virus establishes lifelong latency in dorsal root ganglia (sensory nerve clusters)
- Reactivation triggers: Age-related decline in cell-mediated immunity, immunosuppressive conditions or medications, physical or psychological stress
- Pathophysiology: Reactivated virus travels along sensory nerves to the skin, causing inflammation and characteristic vesicular eruption
- Neural damage: Direct viral injury and inflammatory response cause acute neuritis (nerve inflammation) and potential long-term nerve damage
What are the clinical features?
Herpes zoster presents with a characteristic progression of symptoms:
Prodromal phase (2-3 days before rash):
- Pain and sensory disturbances: Burning, tingling, or stabbing pain along the affected dermatome (nerve distribution area)
- Systemic symptoms: Low-grade fever, malaise (general discomfort), headache
- Localized sensations: Pruritus (itching), allodynia (pain from light touch), hyperesthesia (increased sensitivity)
Acute eruptive phase (7-10 days):
- Vesicular eruption: Grouped vesicles (small fluid-filled blisters) on an erythematous base (red background)
- Dermatomal distribution: Unilateral (one-sided) rash respecting the midline, most commonly affecting thoracic dermatomes (trunk, 50-60% of cases)
- Evolution pattern: Vesicles progress to pustules (pus-filled blisters), then crusts over 7-10 days
- Associated symptoms: Intense neuralgic pain (nerve pain), regional lymphadenopathy (swollen lymph nodes)
Special presentations:
- Herpes zoster ophthalmicus: Involvement of the ophthalmic division of the trigeminal nerve (10-25% of cases), requiring urgent ophthalmological evaluation
- Ramsay Hunt syndrome: Facial nerve involvement with ear pain, facial paralysis, and vesicles in the ear canal
How is it diagnosed?
Diagnosis typically relies on clinical presentation, with laboratory confirmation when needed:
- Clinical diagnosis: Characteristic unilateral dermatomal vesicular rash with associated pain is usually sufficient for diagnosis
- Laboratory confirmation (when clinical diagnosis uncertain):
- Polymerase chain reaction (PCR): Most sensitive and specific test, detecting viral DNA
- Direct fluorescent antibody (DFA): Rapid identification of viral antigens
- Viral culture: Lower sensitivity but high specificity
- Tzanck smear: Shows multinucleated giant cells but cannot distinguish from herpes simplex
- Differential diagnosis: Considerations include herpes simplex virus, contact dermatitis, drug eruptions, and early bullous pemphigoid
What treatment options are available?
Management focuses on viral suppression, pain control, and complication prevention:
Antiviral therapy (most effective within 72 hours of rash onset):
- Valacyclovir: 1000 mg three times daily for 7 days (preferred for compliance)
- Famciclovir: 500 mg three times daily for 7 days
- Acyclovir: 800 mg five times daily for 7-10 days
Pain management:
- Acute phase: Acetaminophen, NSAIDs, or short-term opioids for severe pain
- Neuropathic pain: Gabapentin, pregabalin, or tricyclic antidepressants
- Topical agents: Lidocaine patches, capsaicin cream (after lesions heal)
Adjunctive care:
