Androgenetic Alopecia - Female Pattern Hair Loss
What is it?
Female pattern hair loss, also known as androgenetic alopecia in women, is a genetically determined, progressive thinning of scalp hair that results from the miniaturization of hair follicles (conversion of thick terminal hairs to thin vellus hairs). Unlike male pattern baldness, women typically experience diffuse thinning over the crown while maintaining their frontal hairline, resulting in a widened central part rather than complete baldness.
Who is affected?
Female pattern hair loss is the most common cause of hair loss in women, with prevalence increasing significantly with age, particularly after menopause.
- Prevalence: Affects 1 in 8 women (12%) aged 20-29; increases to 2 in 5 women (40%) by age 50; approximately 1 in 2 women (50%) experience some degree by age 80

- Genetic inheritance: Complex polygenic trait involving multiple genes; AR gene (androgen receptor) variations increase susceptibility; can be inherited from either parent

What are the clinical features?
Female pattern hair loss presents with characteristic patterns distinct from male pattern baldness:
- Ludwig classification: Grade I - mild thinning visible with widened central part; Grade II - moderate thinning with noticeable decrease in hair density over crown; Grade III - severe diffuse thinning with see-through appearance of scalp
- Clinical features: Progressive widening of the central part (Christmas tree pattern when viewed from above); diffuse thinning primarily affecting the crown and top of scalp; preservation of the frontal hairline (unlike male pattern); miniaturized hairs of varying diameters visible on examination
- Associated findings: Increased hair shedding may be noticed on brush or in shower; hair feels less voluminous and difficult to style; scalp may become more visible in bright lighting or when hair is wet
- Disease progression: Gradual onset over years to decades; rarely progresses to complete baldness; may accelerate during hormonal transitions (menopause)
How is it diagnosed?
Diagnosis is primarily clinical, based on characteristic pattern and history:
- Clinical assessment: Recognition of typical pattern - diffuse crown thinning with preserved frontal hairline; hair pull test usually negative (unlike telogen effluvium); dermoscopy shows hair shaft diameter variability (anisotrichosis) greater than 20%
- Laboratory evaluation: Check ferritin levels (iron stores) - optimal level >70 ng/mL for hair growth; thyroid function tests (TSH) to rule out thyroid disease; androgen levels if signs of hyperandrogenism present (hirsutism, acne, irregular periods)
- Differential diagnosis: Must distinguish from chronic telogen effluvium (diffuse shedding with positive pull test); diffuse alopecia areata (patchy loss, exclamation mark hairs); frontal fibrosing alopecia (receding hairline with scarring)
- Scalp biopsy: Rarely needed but shows miniaturized follicles with normal follicle numbers
What treatment options are available?
Treatment aims to slow progression and stimulate regrowth, with best results from combination therapy:
- Topical minoxidil (first-line treatment): 2% or 5% solution or foam applied once or twice daily; only FDA-approved topical treatment for female pattern hair loss; works by prolonging anagen phase and increasing hair diameter; visible improvement in 4-6 months; must continue indefinitely - hair loss resumes 3-4 months after stopping
- Oral medications: Spironolactone 50-200 mg daily - antiandrogen effect, especially helpful if hormonal component suspected; requires contraception in premenopausal women; oral minoxidil 0.25-2.5 mg daily - emerging treatment with good efficacy and tolerability
- Hormonal therapies: Oral contraceptives with antiandrogenic progestins for premenopausal women; hormone replacement therapy may help postmenopausal women; finasteride generally not recommended for women (pregnancy category X)

- Cosmetic approaches: Hair fibers and concealers for immediate cosmetic improvement; volumizing shampoos and styling techniques; hairpieces or wigs for extensive loss
- Expected outcomes: Treatment response varies - approximately 60% see improvement with minoxidil; combination therapy more effective than monotherapy; early treatment yields better results; maintenance therapy required indefinitely
