Bullous Pemphigoid
What is it?
Bullous pemphigoid is the most common autoimmune blistering disease, where the body's immune system mistakenly attacks proteins that anchor the outer layer of skin (epidermis) to the underlying layer (dermis). This attack causes large, fluid-filled blisters to form on the skin. Unlike many autoimmune diseases, bullous pemphigoid primarily affects elderly individuals and, while it can cause significant discomfort and complications, it has become increasingly treatable with new targeted therapies achieving remission rates of 70-90%.
Who is affected?
Bullous pemphigoid predominantly affects older adults, with incidence increasing dramatically with age.
- Prevalence: Affects approximately 7.6 per 100,000 people annually; incidence has doubled in the past two decades
- Age demographics: Average age at onset is 77-83 years; rare before age 50; incidence increases 300-fold between ages 60 and 90
- Gender distribution: Slight female predominance in some studies, though others show equal distribution
- Geographic variation: More common in Northern Europe and Asia; lower rates in Middle East and Africa
- Risk factors: Advanced age; neurological disorders (dementia, Parkinson's disease, stroke - present in up to 36% of patients); certain medications (diuretics, DPP-4 inhibitors for diabetes, checkpoint inhibitors); skin trauma or radiation
What causes it?
The disease results from autoantibody attack on skin structural proteins:
- Autoimmune mechanism: Body produces IgG antibodies against BP180 (collagen XVII) and BP230 proteins; these proteins are crucial components of hemidesmosomes that anchor epidermis to dermis; antibody binding triggers inflammatory cascade with complement activation
- Triggering factors: Medications - especially DPP-4 inhibitors (gliptins), diuretics, antibiotics, TNF-alpha inhibitors; physical triggers - burns, radiation therapy, UV light, trauma; viral infections may precipitate onset
- Neurological association: Strong link with neurodegenerative diseases suggests shared pathogenic mechanisms; BP180 and BP230 also expressed in central nervous system; may explain 2-3 fold increased risk in dementia patients
- Genetic susceptibility: HLA-DQB1*03:01 allele increases risk; familial cases extremely rare
What are the clinical features?
Bullous pemphigoid typically progresses through distinct stages:
- Prodromal phase (weeks to months): Intense itching without visible blisters; red, eczema-like patches or hive-like welts (urticarial plaques); may be misdiagnosed as eczema or drug reaction
- Bullous phase: Large, tense blisters (1-5 cm) filled with clear or blood-tinged fluid; blisters don't break easily unlike pemphigus; surrounding skin often red and inflamed
- Distribution pattern: Flexural areas most common - inner thighs, armpits, abdomen, groin; can affect entire body including palms and soles; oral involvement in only 10-30% (unlike pemphigus with 50-70%)
- Associated features: Severe itching (pruritus) - often worst symptom; new blisters appear as others heal; healing typically without scarring; may have periods of remission and flare
How is it diagnosed?
Diagnosis requires combination of clinical features and specialized testing:
- Clinical assessment: Typical presentation of tense blisters in elderly patient; positive Nikolsky sign (skin doesn't slough with gentle pressure - unlike pemphigus); distribution pattern and associated severe itching
- Skin biopsy with histopathology: Shows subepidermal blister with eosinophils; inflammatory infiltrate at dermal-epidermal junction
- Direct immunofluorescence (gold standard): Linear deposition of IgG and C3 along basement membrane zone; performed on perilesional skin; positive in 85-95% of cases
- Blood tests: Anti-BP180 and anti-BP230 antibodies by ELISA - levels correlate with disease activity; useful for monitoring treatment response; indirect immunofluorescence on salt-split skin
- Differential diagnosis: Must distinguish from pemphigus vulgaris, linear IgA disease, epidermolysis bullosa acquisita, dermatitis herpetiformis
What treatment options are available?
Treatment has evolved significantly with new targeted therapies showing excellent results:
- Topical therapy (localized disease): Super-potent corticosteroids (clobetasol propionate) - effective for limited disease; applied to entire body in severe cases; fewer side effects than systemic steroids
- Systemic corticosteroids (traditional first-line): Prednisone 0.5-1 mg/kg/day initially; gradual taper over months; effective but significant side effects in elderly
- Steroid-sparing agents: Doxycycline 200 mg/day plus nicotinamide - as effective as prednisone in some studies; methotrexate, azathioprine, mycophenolate mofetil as adjuncts
- Biologic therapies (excellent efficacy): Rituximab - 70.5% complete remission at 6 months, now first-line in some centers; dupilumab - 75% achieving disease control, FDA breakthrough designation; omalizumab for refractory cases
- Emerging treatments: JAK inhibitors (tofacitinib, baricitinib) showing promise; complement inhibitors under investigation; anti-IL-17 and anti-IL-23 in trials
- Prognosis: Mortality has decreased from 40% to 6-10% with modern treatment; most achieve remission within 6-12 months; 30-50% relapse rate but usually milder; quality of life significantly improved with newer biologics
Key points for patients
- Bullous pemphigoid is not contagious and is not caused by poor hygiene or diet
- While it primarily affects older adults, modern treatments are highly effective with less toxicity
- Early treatment prevents complications and improves outcomes significantly
- When to seek medical attention: New blisters appearing, especially if widespread; severe itching not responding to treatment; signs of infection (fever, pus, red streaks); difficulty eating or drinking due to mouth blisters
- Living with bullous pemphigoid: Gentle skin care with mild soaps; avoid trauma to skin; regular monitoring of treatment side effects; maintain good nutrition for healing; join support groups for autoimmune blistering diseases
