Androgenetic Alopecia - Male Pattern Baldness
What is it?
Male pattern baldness, or androgenetic alopecia in men, is a hereditary condition characterized by progressive hair loss following a predictable pattern of frontal and vertex (crown) thinning. This genetically programmed sensitivity to dihydrotestosterone (DHT) causes hair follicles to gradually miniaturize, producing progressively finer hairs until they cease producing hair altogether.
Who is affected?
Male pattern baldness is the most common cause of hair loss in men, affecting the majority of men to some degree during their lifetime.
- Prevalence: Affects 3 in 10 men (30%) by age 30; 1 in 2 men (50%) by age 50; 4 in 5 men (80%) by age 70-80

What are the clinical features?

- Hamilton-Norwood classification: Stage I-II - minimal frontal recession, mature hairline; Stage III - deepening temporal recession creating "M-shaped" hairline; Stage III vertex - crown thinning begins; Stage IV-V - frontal and vertex loss with narrowing bridge between; Stage VI-VII - extensive loss leaving only horseshoe pattern of remaining hair
- Clinical features: Bitemporal recession typically appears first (receding hairline); vertex (crown) thinning develops subsequently; hair becomes progressively finer and shorter before ceasing growth; temporal and occipital areas remain unaffected (donor areas for transplantation)
- Hair changes: Affected hairs show progressive miniaturization; terminal hairs convert to vellus-like hairs; decreased hair density becomes noticeable; scalp becomes increasingly visible
- Disease progression: Gradual onset over years to decades; rate of progression varies - some lose most hair within 5 years, others over 15-25 years; without treatment, progression is continuous
How is it diagnosed?
Diagnosis is typically straightforward based on pattern and history:
- Clinical assessment: Recognition of characteristic Hamilton-Norwood pattern; family history of male pattern baldness; gradual onset and progression over years; dermoscopy shows hair diameter diversity (>20% variation) and miniaturized hairs
- Laboratory tests: Usually unnecessary for typical cases; consider thyroid function if rapid or unusual pattern; iron studies if concurrent diffuse thinning; hormonal evaluation rarely needed unless signs of hormonal abnormality
- Differential diagnosis: Must distinguish from alopecia areata (patchy loss with exclamation mark hairs); telogen effluvium (diffuse shedding with positive pull test); scarring alopecias (permanent follicle destruction)
- Scalp biopsy: Rarely required but shows miniaturized follicles with normal total follicle count
What treatment options are available?
Treatment focuses on slowing progression and promoting regrowth, with best results from early intervention:
- Oral finasteride (first-line treatment): 1 mg daily tablet; FDA-approved since 1997; 5-alpha-reductase type 2 inhibitor reducing DHT by 70%; slows hair loss in 90% and promotes regrowth in 65% at 2 years; most effective for crown, moderate for frontal; results visible after 3-6 months, peak at 1-2 years; must continue indefinitely - hair loss resumes within 12 months of stopping
